Application and Registration Forms

Here you can find links to the various forms required by prospective, new, and returning students. Although these forms can be accessed from elsewhere on the site we thought it would be convenient to make them accessible from a single page as well.

All forms are in Adobe PDF format, and require the Adobe Acrobat Reader to open and print them. Most modern computers come with the Reader pre-installed, but if you do not have it you can get it here for free. If you still have trouble opening or printing the forms, please contact us and we will mail you a printed copy.

Here are some deadlines:

  • For admission to the MFA to begin with the August residency and fall semester, April 1.
  • For admission to the MFA to begin with the January residency and spring semester, October 1.
  • For consideration for a scholarship to begin with the August residency, April 1. You must also apply for admission to the program by that deadline.
  • For consideration for a scholarship to begin with the January residency, October 1. You must also apply for admission to the program by that deadline.
  • For admission to the residency-only non-credit option for the August residency, applications are accepted May 15-June 15 on a first-come, first-served basis. The residency-only option is limited to 15 participants, with no more than 7 in any one genre.
  • For admission to the residency-only non-credit option for the January residency, applications are accepted October 6-November 6 on a first-come, first-served basis. The residency-only option is limited to 15 participants, with no more than 7 in any one genre.
